The CHV – Westminster office has moved to the Panorama Orthopedic medical building across the parking lot to the south of the hospital. Our new address is:

A $50 charge will be applied for all no show visits (unless insurance exempt). If you need to cancel or reschedule your visit, this needs to be completed at least 24 hours in advance.
